SEMEN LEAKAGE FROM THE VAGINA AFTER SEX: WHAT IT MEANS!

Before I start proper, I need to make some clarifications:

1. Semen is a combination of sperms and other body fluids secreted by some special organs of the male reproductive system. Semen is what comes out of the male penis when he ejaculates or releases during sex or masturbation. The sperms in the semen are the ones responsible for pregnancy.

2. While you can have a situation where there are no sperm cells in the semen (a condition known as aspermia), you cannot have sperms without semen. As a matter of fact, sperms make up only a meagre 5% of the total volume of semen ejaculated.

This is normal.

However, there are instances when the sperm concentration is less than 5%- a condition called oligospermia.

3. During sex or sexual activity e.g masturbation, oral sex etc, the man goes through 4 phases of sexual arousal that eventually result in him ejaculating (releasing his dangerous seed) into the woman or whatever else is used to collect it. The phase where this happens is known as orgasm and it is the peak of sexual excitement

Now, just before a man orgasms, the sperms move from the testes and on their way out, seminal fluids from the special glands of the male reproductive system get secreted to add to the sperm and make the bulk of semen. Once this happens, the semen ticket out of the penis like a wicked missile!

4. Also, note that, the thickness or stickiness of the semen is made possible by the presence of sperms. At times, the semen can be too watery or too thick (both of these are very abnormal and can cause infertility) depending on the concentration of sperms in the semen and the frequency of intercourse.
5. Once semen is ejaculated, it takes about 30mins for the sperms in it to liquefy, lose their consistency and become like water. Mind you, the semen is already made of over 90% water so this makes it even worse. When the sperms in it now liquefy, the stickiness and thickness is lost. In normal males, it takes about 30mins for semen to liquefy. By the way, some men have theirs too thick- a condition called semen hyperviscosity, which can cause infertility as well.


IS IT NORMAL FOR SEMEN TO LEAK OUT OF THE VAGINA?

Usually, penetrative vaginal sex results in ejaculation or release of semen in the vagina- that is if the sex is unprotected and reaches an orgasm. Once the semen is released, the healthy sperms in it start moving forward, ascending the genital tract to go and fulfil their divine destiny- pregnancy. That's how it happens.

Now, note that it is the sperms that ascend the genital tract, not the entire volume a semen released.

This is very important.

So, the question of whether or not it is normal for semen to leak out of the vagina after ejaculation is dependent on a few factors. With this in mind, it can be normal and it can be abnormal as well...it all depends.

When is semen leakage normal?

1. When the semen flows out after 30mins of sex. Usually, women who experience this often get alarmed thinking something is wrong. No, nothing is wrong. It is normal for the semen to leak out after awhile (usually after 30mins of sex). During this time, the healthy sperms would have ascended the genital tract leaving behind the unhealthy ones which liquefy and add to the semen volume and leak out.

2. When after ejaculation the man continues thrusting. This can cause semen spillage as well. The ideal thing to do when one is a about to release is to thrust deep and then maintain that still position so that the semen gets deposited very close to the cervix where the sperms can easily ascend into
the womb.

3. Also, there is the effect of gravity. Gravity can exert a downward pull on the semen especially when it is not released high up into the vagina and then woman gets up immediately walking about trying to clean up. That said, there are some instances when this may be normal.


When is semen leakage abnormal?

1. It is abnormal when a couple may have been trying to conceive unsuccessfully for a period of at least 12 months. In other words, if semen is always leaking out, it could be a pointer to the fact that the semen is too watery and not having enough healthy sperms. This can affect fertility and cause infertility.

2. It may be abnormal if immediately after ejaculation it starts leaking out even when the woman is still laying on the bed. In addition, it may be abnormal if it flows out less than 30mins after sex even when the man has taken precautions to make sure that the semen gets deposited high up in the vagina.


THE WAY FORWARD?

Well, this all depends on the couple. Personally, I advise couples, especially when they are having difficulty conceiving, to run some fertility tests and be sure that everything is fine. In this case, the man will have to do a seminal fluid analysis (SFA)- a special test in males for assessing their fertility status. So for me, I always ask couple , "Are you having difficulty conceiving"? If they say yes, then I almost always request that the man gets his SFA done while the woman may need to run some tests as well, depending on my suspicion or diagnostic inclination.
